Today, the word “lobotomy” is rarely mentioned. If it is, it’s usually the butt of a joke.


But in the 20th century, a lobotomy became a legitimate alternative treatment for serious mental illness, such as schizophrenia and severe depression. Physicians even used it to treat chronic or severe pain and backaches. (As you’ll learn below, in some cases, there was no compelling reason for the surgery at all.) There’s a surprising history of the lobotomy for its use in mental health.


A lobotomy wasn’t some primitive procedure of the early 1900s. In fact, an article in Wired magazine states that lobotomies were performed “well into the 1980s” in the “United States, Britain, Scandinavia and several western European countries.”


The Beginning


In 1935, Portuguese neurologist Antonio Egas Moniz performed a brain operation he called “leucotomy” in a Lisbon hospital. This was the first-ever modern leucotomy to treat mental illness, which involved drilling holes in his patient’s skull to access the brain. For this work, Moniz received the Nobel Prize in medicine in 1949.


The idea that mental health could be improved by psychosurgery originated from Swiss neurologist Gottlieb Burckhardt. He operated on six patients with schizophrenia and reported a 50 percent success rate, meaning the patients appeared to calm down. Interestingly, Burckhardt’s colleagues harshly criticized his work at the time.


The Lobotomy in America


In 1936, psychiatrist Walter Freeman and another neurosurgeon performed the first U.S. prefrontal lobotomy on a Kansas housewife. (Freeman renamed it “lobotomy.”)


Freeman believed that an overload of emotions led to mental illness and “that cutting certain nerves in the brain could eliminate excess emotion and stabilize a personality,” according to a National Public Radio article.


He wanted to find a more efficient way to perform the procedure without drilling into a person’s head like Moniz did. So he created the 10-minute transorbital lobotomy (known as the “ice-pick” lobotomy), which was first performed at his Washington, D.C. office on January 17, 1946.


(Freeman would go on to perform about 2,500 lobotomies. Known as a showman, he once performed 25 lobotomies in one day. To shock his audiences, he also liked to insert picks in both eyes simultaneously.)


According to the NPR article, the procedure went as follows:


“As those who watched the procedure described it, a patient would be rendered unconscious by electroshock. Freeman would then take a sharp ice pick-like instrument, insert it above the patient’s eyeball through the orbit of the eye, into the frontal lobes of the brain, moving the instrument back and forth. Then he would do the same thing on the other side of the face.”


Freeman’s ice-pick lobotomy became wildly popular. The main reason is that people were desperate for treatments for serious mental illness. This was a time before antipsychotic medication, and mental asylums were overcrowded, Dr. Elliot Valenstein, author of Great and Desperate Cures, which recounts the history of lobotomies, told NPR.


“There were some very unpleasant results, very tragic results and some excellent results and a lot in between,” he said.


Lobotomies weren’t just for adults either. One of the youngest patients was a 12-year-old boy! NPR interviewed Howard Dully in 2006 at the age of 56. At the time, he was working as a bus driver.

STATEN ISLAND, N.Y. — As their hard-earned friends in general education are already imagining life next year at Paulo Intermediate School, the fifth graders with Asperger’s Syndrome in the first graduating class from PS 4’s Nest program have yet to learn where they will spend their adolescent years.


For these youngsters with autism, who are prone to struggle with even the smallest transitions, it is unacceptable not to be heading into adolescence with familiar faces from elementary school, said parents, who are demanding the link to Paulo be established now and maintained for future classes.


Shouting “Nest is best” and “Our students will thrive at IS 75,” nearly 100 parents from the program and their supporters today gathered outside the Arden Heights school alongside Public Advocate Bill DeBlasio, state Sen. Andrew Lanza (R-Staten Island) and City Councilman Vincent Ignizio (R-South Shore) to push the city Department of Education to quickly make what they characterized as an intuitive, humanistic and educationally sound decision.


“Middle school can be a land mine for even the most well adjusted child. For a child with any social deficiency it can be ten times as difficult to navigate,” said Terry Ballou, whose fifth-grade son, Ryan, has been in the Nest program since kindergarten.


“We want continuity. We want our children with their friends they’ve worked so hard to bond with over these six years,” said Denise Ingrassia. “Our children need to be primed to transition,” she said, describing her son, one of six fifth-graders in the school with Asperger’s in the school’s Nest program.


Most children with Asperger’s Syndrome perform on grade level and are often intellectually gifted, but require instruction in the every day interactions that come as second nature to most people, such as making eye contact and engaging in the give-and-take of conversation.

When Jacob Barnett first learned about the Schrödinger equation for quantum mechanics, he could hardly contain himself.


For three straight days, his little brain buzzed with mathematical functions.


From within his 12-year-old, mildly autistic mind, there gradually flowed long strings of pluses, minuses, funky letters and upside-down triangles — a tapestry of complicated symbols that few can understand.


He grabbed his pencil and filled every sheet of paper before grabbing a marker and filling up a dry erase board that hangs in his bedroom. With a single-minded obsession, he kept on, eventually marking up every window in the home.


Strange, say some.


Genius, say others.


But entirely normal for Jacob, a child prodigy who used to crunch his cereal while calculating the volume of the cereal box in his head.


“Whenever I try talking about math with anyone in my family,” he said, “they just stare blankly.”


So do many of his older classmates at Indiana University-Purdue University Indianapolis, who marvel at seeing this scrawny little kid in the front row of the calculus-based physics class he has been taking this semester.


“When I first walked in and saw him, I thought, ‘Oh my God, I’m going to school with Doogie Howser,’ ” said Wanda Anderson, a biochemistry major, referring to a television show that featured a 16-year-old boy-genius physician.


Elementary school couldn’t keep Jacob interested. And courses at IUPUI have only served to awaken a sleeping giant.


Just a few weeks shy of his 13th birthday, Jake, as he’s often called, is starting to move beyond the level of what his professors can teach.


In fact, his work is so strong and his ideas so original that he’s being courted by a top-notch East Coast research center. IUPUI is interested in him moving from the classroom into a funded researcher’s position.


“We have told him that after this semester . . . enough of the book work. You are here to do some science,” said IUPUI physics Professor John Ross, who vows to help find some grant funding to support Jake and his work.


“If we can get all of those creative juices in a certain direction, we might be able to see some really amazing stuff down the road.”


“My fear was that he would never be in our world”


Teenage college student?


Developer of his own original theory on quantum physics?


Paid researcher at 13?


This is not what Jake’s parents expected from a child whose first few years were spent in silence.


“Oh my gosh, when he was 2, my fear was that he would never be in our world at all,” said Kristine Barnett, 36, Jake’s mother.


“He would not talk to anyone. He would not even look at us.”


Child psychologists assessed Jake at the time and diagnosed behavioral characteristics of a borderline autistic child. He was impaired, they said, and had a lack of “spontaneous seeking to share enjoyment,” difficulty showing emotion and interacting with others.


Diagnosis: mildly autistic.

Happiness. Ahh, what an enticing word that is.


Psychologists call it “subjective well-being” (and even abbreviate it as SWB in their research), but it boils down to the same thing — what makes us more happy? And how can we do more of that special stuff that will lead to greater happiness in our lives?


This Emotional Life, a new PBS documentary hopes to help answer that question in three 2-hour shows from January 4 through the 6th, 2010. “Each episode weaves together the compelling personal stories of ordinary people and the latest in brain science research, along with revealing comments from celebrities such as Chevy Chase, Larry David, Elizabeth Gilbert, Alanis Morissette, Katie Couric and Richard Gere.” Sounds like good stuff and we’re happy to help promote this new series (I also am blogging on their website!).


We’ve previously noted how at least 40% of our happiness is determined by our own behaviors, and how money can lead to happiness but there’s a catch — it helps to give your money away. The more you give away, the happier you might be.


We’ve also noted how your happiness is affected by being in a relationship. Partners are more happy when they help the other achieve his/her dreams. Another blog entry we’ve written discusses the keys to happiness in a long-term relationship:


  • *Continue exploring and engaging in new activities and opportunities with one another.
  • *Resolve conflicts as they occur, respectfully; don’t let conflicts simmer and build into something bigger over time.
  • *Find something challenging to do together; a shared challenge can help bring you closer together if you work as a team.
  • *Feeling that your partner is “there for you” is invaluable for a good relationship; loyalty counts more than you may realize.
  • *Guard against insecurity, as it can lead to spark relationship problems that otherwise wouldn’t exist.

 

Happiness is not only possible for most of us — it’s an easy opportunity that’s waiting for us in every decision we make, every action we take. Be kind to others, give generously, and respect and be loyal to those you most love. It may take some practice, but you might find that happiness is not just within reach — it’s within your life too!

A couple of weeks ago, I begrudgingly attended a singles event at a local museum. Depression can often keep me safely tucked away in my apartment, but in the spirit of not wanting to further cultivate my reputation of being a stick in the mud, I went. Besides, visions of fancy steak on a stick h’or dourves and a complimentary coat check danced in my head.


Leslie works as a Marketing and Training Director in the property management field. She shares her home in southern New England with a somewhat obstinate but adorable dog, Monster.

Greetings!

Last week, the ICD-9-CM Coordination and Maintenance Committee met to discuss the future of medical coding in the United States. The ICD-9-CM stands for the International Classification of Diseases, Ninth Revision, Clinical Modification, and is the US government’s official system of assigning codes to medical diagnoses and procedures. The day before the meeting, the Centers for  Disease Control and Prevention (CDC) posted for the first time information on the codes under consideration – including a new medical diagnosis for “wandering” related behavior in children and adults on the autism spectrum and with other developmental disabilities. If approved, this new coding promises to label hundreds of thousands of children with “wandering” diagnoses that would make it easier for school districts and residential facilities to justify restraint and seclusion in the name of treatment. Furthermore, this diagnosis carries no clear definition and the CDC’s proposal uses poor quality research to claim that it should apply to the majority of autistic children and those with other developmental and intellectual disabilities.

 

The CDC’s last minute proposal was made public only the day before the public hearing on these coding was scheduled to occur – well after the registration for people to give public comment had closed! Our only chance to have our voices be heard is to flood the written comment session before that deadline passes on April 1st. To do that, we need your help! Here’s what you can do:

 

  1. First, send an e-mail to to CDC’s co-chair of the ICD-9-CM Coordination and Maintenance Committee telling them to REJECT a medical label for “wandering”-related behavior. Her name is Donna Pickett and her e-mail is dfp4@cdc.gov. Remember, they have to hear from us by April 1st! We’ve provided some talking points to help you make your case:
    • Labeling hundreds of thousands of children with a “wandering” diagnosis will increase restraint and seclusion in schools: One of the consistent messages from our community in last year’s advocacy for federal legislation to stop restraint and seclusion in schools was that when schools plan to restrain students, they do restrain students – frequently with tragic results. By labeling hundreds of thousands of school children with disabilities with a diagnosis of “wandering”, CDC will encourage districts to plan for the use of restraint for these students in Individualized Education Plans (IEPs) and school safety planning. Furthermore, by claiming that “wandering” is an unavoidable medical diagnosis instead of a behavioral response to specific circumstances, children with little to no communication needs may lose one of their last ways of making family members and educators aware of abusive or sensorily overwhelming environments: trying to leave a dangerous situation. Far from making children with disabilities safer, this proposal will enable abuse “in the name of treatment” and make it harder for non-speaking students to communicate problems to their families.
    • The “wandering” diagnosis lacks meaningful research support: There exists no research to classify “wandering” as a medical rather than a behavioral issue. This proposal is being pushed forward without meaningful research support. In fact, one of the few “studies” that the CDC does site is a 2007 online poll on the website of an advocacy group in support of this proposal claiming that this code could apply to as many as 92% of autistic children. An online poll on a web site mainly visited by supporters of the proposal is not a scientifically valid survey instrument.
    • The use of the “wandering” label on adults will enable abuse and restrict the civil rights of Americans with Disabilities: As children labeled with this diagnosis grow up, a “wandering” label could be used as a factor to justify guardianship – the stripping of legal capacity – in areas where it otherwise would not be deemed acceptable. Advocates of a “wandering” label make the case that its usage would enable insurance coverage for tracking devices, whose use for adults would restrict freedom of movement and make it harder for individuals to flee abusive situations. Furthermore, this diagnosis will increase the usage of more restrictive service-provision placements, like institutions and group homes, as a way of countering the “flight risk” that labeled individuals will be presumed to pose.
  2. Sign ASAN’s petition to tell CDC to reject and withdraw this ill-considered proposal to create a “wandering” medical diagnosis. A link can be found at:http://www.change.org/petitions/tell-the-cdc-no-on-abuse-enabling-wandering-code

  4. Disseminate this action alert to your friends and family members as well as members of local, state and national advocacy groups you may belong to who can pass it along to their members. Regular use of antipsychotic and antidepressant medication has increased among residents of Sydney nursing homes since 2003, an expert says.


In a letter published in the latest Medical Journal of Australia, Clinical Professor John Snowdon and co-authors also reported that regular use of anxiolytic and hypnotic medication has decreased since the 1990s. They compared their 2009 study into psychotropic medication use in Sydney nursing homes with similar studies conducted in 1993, 1998 and 2003.


Prof Snowdon said that the use of antipsychotic agents fell between 1993 and 1998 but by 2009 there had been a progressive change from conventional antipsychotics to a three-to-one preference for atypical antipsychotic medication. By 2009, the rate of regular use of antipsychotic medication had risen above the level it had been in 1993.
